Husband is/was sole provider. His Primary Care Physician  placed him on medical short term disability and FMLA. For three months at least. He is not able to drive nor work without falling asleep. He also has back pain, that was just told that he may need surgery.  I have to take him to his appointments.  Th8s hindering my work abilities.  We need assistance in paying  for our December and January rent at least. We are fighting to get his short term disability at least. We are also fighting for long term disability.  This does take time for both. My husband is on a sleep machine and has air purifiers through the home. 

1. We have received an pay or quit notice for December.
