Rent Assistance

Good Afternoon! I am in need of rental assistance for the month of November and December.  I had to take leave from work when my husband was admitted into the hospital at Chandler Medical Center. I did not know that when I took leave from work it was unpaid leave.  I was not told until the end of November. After my husband was discharged from the hospital he was admitted in a rhab facility.  But because of a misunderstanding between the doctor from Chandler and the rehab facility I had to make time to obtain medical records and orders for a machine my husband needed.  Which required me to either download forms and fax them back or drive to the nearest hospital to sign Release of information forms.  I am also trying to get the house ready for my husband’s return once he is discharged from the rehba facility.  I won’t be able to return to work full time but part time until I can get all my husband’s appts in order.
